How to Access Work Permits and Proof of Enrollment Forms

Summertime is approaching and some of you will need to get your work permits or proof of enrollment forms.  This is a reminder that Mrs. Roux is still here processing work permits and proof of enrollment forms for the DMV. Here is how the process has changed

Work Permits

Work permits cannot be done in person right now, so, you will have to give yourself a few days for this process to be completed. 

Step 1- Send me a completed Application to Employ Minors Under Age 18. If you do not have this form, please e-mail me and I will be happy to send you the form. 

Step 2 - Once the form is received, I will process it for the work permit. I will e-mail the permit back to you for your signature. 

Step 3 - Please scan the completed work permit back to me via e-mail, for my records. The process is not completed until I receive the signed form back. If you do not have access to a scanner but have a cell phone, there is an app that you can download to your phone. It is called PDF Scan or Scanner (Adobe PDF Scan).

*If you will be working as a lifeguard, please send a copy of your CPR certification with the application to employ a minor. 

Proof of Enrollment Forms

Just like the work permits, the proof of enrollment forms cannot be done in person right now. You will need to give yourself a few days prior to visiting the DMV or a title company for this process to be completed. 

Step 1 - Send me an email requesting the form. Include your full name (first, middle, and last), address, and date of birth.

Step 2 - I will put this in the mail for you and you will receive it when the regular mail delivers. 

Step 3 - This form is good for 90 days. You will need to give yourself time to receive it and make sure that if you have a form that is about to expire, that you request a new one.
